How to Register and Login to Power2Patient: AMI’s Online Portal
We use an electronic medical record program (EMR) called Power 2 Patient. It is through a portal that you will be able to view portions of your chart and through with your physician and team of specialists at Eternity Health Partners will send you such items as messages, lab requisitions, and reminders for follow-up care and results of recent studies.
How to Register to Power2Patient
Step 1: You will receive an email from Eternity Health Partners with directions on how to Register to Power 2 Patient (P2P).
Step 2: Click the yellow bar “CLICK HERE!” in your email.
Step 3: Register Here
Provide your username (which will be the email you provided Eternity Health Partners)
And your Email address below
Click “SUBMIT”
Step 4: Under Patient Registration fill out the following information;
Date of Birth, First Name, Last Name, Gender
Step 5: Under Patient Registration fill out the following information;
Create New Password
Click Save
Step 6: Answer 3 Security Questions
Click Save
How to Login to P2P
Step 1: Login to Power2Practice www.Power2Patient.net
Step 2: Enter Username
Step 3: Enter Password
Step 4: Click Sign In
If you have not received the onboarding email from customerservice@support@power2patient.com
with the subject: “Power2Patient New Patient Registration”, please contact our office at 805-608-4582.
NOTE: No outside person or entity can access the program, your medical records are entirely private.
This program works best by using Google Chrome or Firefox. You must have a private email address, separate from a spouse or family member for HIPAA compliance.
You can view your chart at any time by logging onto www.power2patient.net which can also be access by clicking client portal on our website. If you forget or lose your password you can reset it at any time through this website.
Notifications of letters, messages and documents from this office will come to your email address. Clicking on the email notification will take you to the message or document within your Power2Patient Chart.
Also please be aware that there is no way for the patient or outside entity to add documents to the chart via email or through the EMR program. If you or someone on your behalf has documents to be added to your chart, the document must be brought to the office for scanning or faxed to the office for inclusion into the chart.
